In addition to the information required to be included in the written statement of the applicant for a first license, and of the licensee for a renewal of license, as provided in Section 9.04.070, and in addition to and as a part of the procedure to be followed in any assignment or transfer of a license, as provided in Section 9.04.095, each applicant, licensee or transferee who is subject to the contractors license law shall state that they are licensed under such law, that their license is in full force and effect, and the number thereof, or, if they are exempt from the provisions of the contractors license law, proof of the facts which entitle the applicant to such exemption.
